Analysis of Film Set Injuries and Deaths (2002-2024)

The motion picture and video industries play a significant role in entertainment and culture within the US, producing films, television shows, video games, and various forms of digital content.  These industries are categorized under NAICS 5121 and encompass a wide range of activities from pre-production planning to post-production editing, distribution, and viewings in theaters. Motion picture production in particular involves a large crew working from on-set locations to production studios, each presenting its own set of risks.

Despite the glamour associated with Hollywood and filmmaking, the industry is a high-stakes environment where safety remains a constant concern. With pressure to meet strict deadlines, perform perilous stunts, and operate heavy equipment, the risks of a catastrophic or even fatal accident remains high. This analysis by our Los Angeles workers’ compensation attorneys details catastrophic injuries and fatalities from 2002 to 2024 as a result of a workplace hazard. The report from The Occupational Safety and Health Administration (OSHA) defines fatalities and catastrophes as follows:

• Fatality. An employee death resulting from a work-related incident or exposure; in general, from an accident or an illness caused by or related to a workplace hazard.
• Catastrophe. The hospitalization of employees resulting from a work-related incident or exposure; in general, from an accident or an illness caused by a workplace hazard.

Given the prominence and sheer scale of the film industry, it’s important to analyze the workplace hazards that can lead to a traumatic event.

How Many Incidents Have Occurred on Film Sets?

Since 2002, there have been 152 separate incidents resulting in catastrophic injury or death. With a total of 16, 2019 had the most reported incidents in this time period. Following the October 2021 Rust incident involving Alec Baldwin, rates have fallen significantly- perhaps due to increased scrutiny on the industry.

How Many Deaths Have Occurred on Film Sets?

There have been 32 deaths between 2002 and 2024. Excluding deaths from distribution and theaters, film sets lead the way with 22 fatal incidents. The deadliest year within the scope of this study was 2011  with a total of 5 deaths- 4 of which were the result of falls.

At 22 deaths, film sets account for 69% of fatalities in this industry while theaters and distribution have 9 and 1 deaths, respectively.

Who Gets Injured the Most On Film Sets?

Of incidents with a reported job title, those with job titles categorized as “Laborers, except construction” were the most likely to be killed or injured on film sets, followed by “actors and directors” and “artists, performers and related workers.”

Laborers, often referred to as set workers, are typically responsible for tasks such as setting up production equipment, installing and moving props, and rigging and dismantling heavy equipment such as scaffolding, platforms, and backdrops. Compared to other job duties, this puts them at a unique risk for serious accidents like slips, falls, and crushing injuries.

Abstract: At 6:50 a.m. on February 6, 2024, an employee was working as an engineering technician for a television and film production company. The employee was working with a crew which included Employee #1 and Employee #2. The crew was working on wooden catwalks located in Stage 3. The crew was collecting and removing lighting cable equipment that was hanging from the wooden platforms. One platform was accessible via a wooden staircase that continued upward and was connected to the other wooden catwalk platforms. The walking surface of these catwalks consisted of wooden floorboards of various sizes, including as 2-inch by 6-inch and 2-inch by 8-inch boards. These floorboards were supported underneath by 2-inch by 4-inch wooden supports (sometimes referred to as “ledgers”). A particular section of this catwalk had a deteriorated ledger that was not bearing the 4-inch by 12-inch underlying support beam, but instead was improperly nailed to a roof truss. The deteriorated ledger was weakened likely due to age, environmental conditions and repeated stress loads over many decades. As the crew walked along the catwalks, the employee stood on the weakened section of catwalk. Suddenly, and without warning, the ledger supporting the floorboard broke apart and collapsed as the employee was standing on this section of catwalk. The employee fell 41 feet and forcefully impacted the ground below. The employee was pronounced dead by responding emergency medical services shortly after the fall. The cause of death was blunt force trauma to the head, neck and legs.

The vehicle lost traction, slid across a two lane highway, tumbled, caught fire and came to a stop overturned approximately 300 feet from the road surface. The employee survived the crash but was killed by the fire.

The coworker fired the weapon discharging a projectile which struck both employees causing severe injuries. Employee #1 incurred a gunshot wound her right armpit/chest/abdoman areas and upper middle back as the bullet exited her body killing her. Employee #2 incurred a gunshot wound to right shoulder, requiring hospitalization. No projectile was supposed to be in the firearm, which should have only container a dummy round with no gunpowder. The investigation is ongoing to determine how live rounds ended up in the revolver.

While filming on the train trestle, a train came on the track. Due to the lack of time and a safe method to exit the trestle, the employee and eight coworkers were not able to exit the trestle prior to the train reaching them and, therefore, huddled on the east side of the trestle on a metal walkway platform. Other employees pulled the guardrail back to dislodge the lift. When Employee #1 drove forward, the lift went off the side of the ramp and fell approximately 5 feet to the concrete floor. Incident Summary: On June 13, 2011, Employee #1, a maintenance worker, was cleaning a sign that was damaged by fire earlier. While the floor was still wet from extinguishing the small fire, Employee #1 used a metal extension ladder to access the sign. The ladder slipped, and Employee #1 fell to the tile floor below and suffered severe head injuries. Employee #1 was hospitalized and later died. Further investigation recorded that the angle of the ladder was approximately 64 degrees which was too flat and not in compliance with the 4 to 1 (75.5 degrees) requirement of 1910.26(c)(3)(i). No one was holding the ladder for Employee #1. The ladder was not tied off or secured. The floor had been washed and was slippery in violation of 1910.26(c)(3)(iii) .

Near the lamp he observed an electrical cord hanging. He used a 12 ft fiberglass ladder to gain access to the electrical cord. Upon examining the electrical cord it turned out to be a rope. He descended the ladder and on the tenth or ninth rung, missed a step, lost his balance, and fell. He was hospitalized with a compound fracture to his left elbow.

Incident Summary: On October 14, 2005, Employee #1, a printing press operator, was working in the studios’ print shop in building Number 89. He was printing letterheads and envelopes on a Hamada 665CD Offset Press. According to the employer’s representatives and the County of Los Angeles Coroner’s report, Employee #1 reported to the paramedics, who responded to the accident, that he became dizzy and struck his face on the metal trash can when he fell to the ground. Employee #1 was taken to Brotman Medical Center, where he was diagnosed with an acute myocardial and an acute frontal brain hemorrhage. The surgery was performed for the intracerebral bleeding, but Employee #1’s functions continued to deteriorate. He was pronounced dead on November 1, 2005. The coroner listed the cause of death as blunt force head trauma with intracerebral hemorrhage. In addition, the coronary artery disease contributed to the immediate cause of death.

A coworker was operating a red 1999 Ford F-550 truck (Serial Number 5V26838) transporting a generator. After arriving at the parking lot, the coworker unhooked the equipment in one of the parking spaces and started to approach another coworker in the backward direction. Incident Summary: On October 29, 2002, Employee #1, an actor, made a parachute jump from 14,000 ft during the filming of a military-themed movie in Oceano Dunes State Park, CA. He landed in the water instead of the beach as planned. A rescue attempt with personal watercraft was unsuccessful, and his body was found three days later. The jump had been practiced on the ground and a practice jump had been successfully executed that morning. A safety conference had been held and an emergency action plan was written, but not followed.
